Charitable Request Donation Form
* The necessary donation form must be completed in its entirety a minimum of four weeks prior to the requested deadline.
* Football vouchers for tickets are not given as a donation.
* Pursuant to NCAA regulations, items will not be donated to any entity that benefits a high school, high school-aged student(s) or affiliates of any high school-aged students.
* If the donation is for an individual's benefit and there are children in the recipient's family, you must provide ages or grades of all children. If any are within grades 9-12, the request will be denied.
* BGSU Athletics items will not be donated to other colleges or universities.
* Limit one request per event.
* BGSU Athletics reserves the right to select the game(s) to which vouchers will be granted.
* For any autograph-requested item (i.e. basketball, football, hockey stick), you must provide the item.
* Student-athlete autographs require additional NCAA Paperwork (NCAA Bylaw 12.5.1.1 Promotional Activities form).
* No high school requests will be filled.
