We also need a copy of the high school's Certificate of Liability Insurance Form.
INDIVIDUALS: Individuals must complete the entry form located in the Sid Sink Individual Entry Form site; this form must be completed in its entirety and times confirmed by your high school coach. We also need you to read and sign the individual liability for and have your parents sign the form if you are under 18 years old.
